If you need to show paperwork for a change of name or to get remarried, you will need a certified copy of the VS (vital statistics) form. To obtain a certified copy from the court, fill out the Request for Certified Copy of Divorce Decree or Vital Statistics and send the completed form, along with $5, to the court.
If you need a certified copy of the divorce and all the related paperwork, fill out the Request for Certified Copy of Divorce Decree or Vital Statistics form and send the completed form, along with $25, to the court.
Please Note: It is recommended that you send a cashier's check or a money order as in some courts your personal check may be held until it clears from the bank.
